caring@home Community Palliative Care Resources

Tailored resources for families, health professionals and clinical services to support the provision of quality end-of-life care for people who choose to be cared for at home.   

These resources are packaged as a Community Palliative Care Resources Box. Detailed information about the contents of the Box and how to order are available here. 

The Resources Box is designed to be stored centrally and contains resources that can be used with several patients and families. Clinical services may order ONE RESOURCES BOX PER SITE.

Printed resources contained in the Resources Box can also be downloaded and printed to restock the Box. (see below)

PLEASE NOTE: It is very important to note that the clinical processes of carers managing subcutaneous medicines at home have not changed as they continue to be consensus-based best practice. Existing resources in individual caring@home packages for carers can still be used.

The caring@home Matrix document (1MB pdf) guides health professionals to choose resources to best suit families, carers and patients.

** It is recommended when providing resources to families, that they are printed in colour.
